Welcome to the Company Address change service for the Indiana Department of Insurance (IDOI). Changes made using this service will automatically update the official address record for your company maintained at the IDOI. Only certain address types are modifiable using this service. Address changes that are not modifiable through this service must be communicated to the state using the Uniform Certificate of Authority Application (UCAA) Form 14.

Upon initial logon the only address type available for modification is for PIN contact and address for verification or modification. Logon to this service is required for PIN contact verification or PIN contact modification.
