Changes made using this service will automatically update the official address record for your company in our database. Only certain address types can be changed using this service. Addresses that are not modifiable through this service must be changed by communicating to the State using the NAIC's Uniform Certificate of Authority Application (UCAA), form 14.

If you have not provided the Insurance Division with an address for a specific address type in the past, you will not be able to add it using this service. Please contact the Department directly. Once the address type is added, you will be able to modify it using this service.
